At Apple Hill Hospital, we understand that accessing mental health care is a crucial step for many individuals facing mental health challenges. Whether you are seeking help for yourself, a loved one, or on behalf of a patient or client, we are here to support you through the referral process.

We take pride in offering a streamlined and compassionate approach to referrals, ensuring that everyone who needs our services can easily access the specialist care they deserve. Our referral process is designed to cater to both NHS referrals and private referrals, allowing us to support a wide range of individuals in need.

Who Can Refer to Apple Hill Hospital?

We accept referrals for both private and NHS patients, ensuring that individuals in need of specialist mental health care can receive the support they require. Whether you are a healthcare professional, a family member, or an individual seeking help for yourself, we are here to guide you through the referral process.

For private patients, referrals can be made by family members, healthcare professionals, or the individual themselves. We understand that seeking mental health care can be a significant decision, and we aim to provide flexibility in our admissions process to make it as smooth and accessible as possible. Private referrals allow for quicker admission, as there is no need to wait for NHS funding approval, and the care provided is tailored to meet the specific needs of the individual.

For NHS patients, referrals can be made through the patient’s GP, mental health team, or other healthcare professionals involved in their care. We work closely with the NHS and local Integrated Care Boards (ICBs) to ensure that patients receive timely access to the care they need. Our team is experienced in collaborating with NHS services, including community mental health teams (CMHTs) and crisis teams, to ensure that patients are transferred to our care as smoothly as possible.

NHS Continuing Healthcare (CHC) Funding

For individuals with complex mental health needs that require ongoing psychiatric rehabilitation, NHS Continuing Healthcare (CHC) funding may be available. CHC is a fully funded package of care provided by the local ICB, designed to support individuals who have ongoing healthcare needs that cannot be met through standard NHS services. This funding is available to individuals who meet specific criteria, and eligibility is determined through a thorough assessment of the patient’s health condition and care requirements.

Not all patients will qualify for CHC funding, but for those who do, it offers a valuable option for accessing specialist mental health care without the financial burden. Our team at Apple Hill Hospital is here to support healthcare professionals, families, and patients in navigating the CHC referral process. We can assist with the necessary documentation and work with the local ICB to ensure that all relevant information is submitted to support the patient’s application for CHC funding.

Our Referral Process

At Apple Hill Hospital, we aim to make the referral process as simple and straightforward as possible. We believe that accessing mental health care should not add unnecessary stress to individuals or their families, and we are committed to guiding you through each step.

NHS Referrals

For healthcare professionals referring patients through the NHS, we have a dedicated referral team that liaises with NHS services, local authorities, and ICBs to ensure a smooth transition of care. Our team works closely with GPs, mental health teams, social services, and other professionals to ensure that patients receive the appropriate level of care in a timely manner.

  • Initial Assessment: When we receive an NHS referral, our team will carry out an initial assessment to determine the individual’s needs and the most suitable care pathway. This assessment may include a review of medical history, mental health assessments, and discussions with the referring professional.
  • Collaboration with NHS Services: We work in close collaboration with NHS mental health teams, including crisis teams, community mental health teams (CMHTs), and inpatient services. This ensures continuity of care and that the individual’s treatment plan aligns with NHS guidelines and care standards.
  • Admission and Care Plan: Following the initial assessment, our clinical team will develop a comprehensive care plan tailored to the individual’s specific needs. We will then work with the referring NHS professional to ensure that the individual is transferred to our care seamlessly. Our team will provide updates on the individual’s progress and work collaboratively with the NHS team to facilitate discharge planning when appropriate.
  • Follow-up and Aftercare: We believe in providing ongoing support beyond the initial treatment phase. Our aftercare services are designed to support individuals as they transition back into the community, working closely with NHS services and local mental health teams to ensure a smooth and supported recovery journey.

Private Referrals

If you are making a private referral, either for yourself or on behalf of someone else, our admissions team is available to guide you through the process.

  • Contacting Us: The first step in making a private referral is to contact our admissions team. You can reach us by phone, email, or by filling out our online referral form. During this initial contact, we will gather some basic information about the individual’s mental health needs and discuss the available treatment options.
  • Assessment and Consultation: Once we receive your referral, our clinical team will arrange a thorough assessment, either in person or remotely, depending on your preference. This assessment allows us to understand the individual’s mental health challenges and tailor a treatment plan accordingly. Our assessments are conducted by experienced mental health professionals, including psychiatrists, psychologists, and nursing staff.
  • Treatment Options: Based on the assessment, we will recommend a personalised treatment plan that may include inpatient care, outpatient therapy, or other specialised interventions. We will provide full transparency on the costs involved and work with you to ensure that the treatment is affordable and accessible.
  • Admission: If inpatient treatment is required, we will arrange a suitable admission date and ensure that all necessary arrangements are made to facilitate a smooth transition to our care. We understand that this can be a challenging time, and our team is on hand to provide support and reassurance throughout the process.
  • Family Involvement: At Apple Hill Hospital, we value the involvement of family members and loved ones in the individual’s care. With the individual’s consent, we encourage family involvement in the treatment process and provide regular updates on progress. We also offer family therapy sessions where appropriate to support the individual’s recovery journey.
  • Discharge and Aftercare: Upon completing their treatment at Apple Hill Hospital, individuals will receive a comprehensive discharge plan. This plan may include recommendations for ongoing therapy, medication management, and support services. Our aftercare services are available to all individuals who have been treated at our hospital, ensuring that they have the support they need as they reintegrate into their daily lives.
